Describe a situation the clinician can picture
“Conversation is difficult” can mean several things. Describe whether it happens in a quiet room, with several people talking, over the phone, or when someone speaks from another room.
Add the practical effect: asking for repetition, turning one ear toward a speaker, or missing part of a meeting. These are observations for discussion, not a diagnosis.
Preserve the test conditions
If you used an app, note the headphones, surroundings, and approximate settings. Home screening depends on equipment and conditions and should not be treated as equivalent to a calibrated clinical assessment.
Keep the exported result rather than rewriting its score in a notebook without context. Label it as an app screening.
Example note: difficulty following two speakers at dinner; easier in a quiet room. First noticed several weeks ago. App screening used ordinary headphones in the living room.
Prepare questions you need answered
Ask what the result means, whether further testing is needed, and how to compare future results. If work or daily tasks are affected, mention those situations explicitly.
Hearing Test & Audiogram offers screening records and export features. Its App Store description states that it is not a medical device and cannot diagnose hearing loss.

Do not wait when hearing changes suddenly
NIDCD treats sudden sensorineural hearing loss as a medical emergency. A sudden change should prompt immediate medical evaluation, even if it seems like wax or congestion. Do not spend time repeating a home test first.
